How to Apply

The College has a paper application process. Please review the registration requirements applicable to all applicants, then follow the application process appropriate to your situation:

If you determine that you meet the requirements, you’re ready to apply for registration with the College. Complete the appropriate application form, gather the required documentation, and submit your completed application to the College.

Original documents should be mailed to the College:

Application forms and document copies can be mailed to the above address, emailed to or faxed to 780.432.9106.

Please note that if you plan on practicing in more than one specialty, you’ll need to submit a separate application for each one. You will also need to submit an Additional and Enhanced Practice Authorization Application if you will be performing certain restricted activities.

Once you’ve submitted your application, the College will review it within three to five business days and contact you with the next steps.
